Scientific visit to the VFN hospital pharmacy from the Netherlands

On April 21, 2023, a pharmaceutical scientific visit came to the hospital pharmacy. The hospital pharmacy, together with other departments of the hospital, prepared an all-day program for her. The symposium was divided into two parts. The morning joint lectures took place in the wonderful historic Charvát hall, the richly stucco-decorated upper part of the former chapel of St. František Xaversky. The afternoon part was organized interactively, forty participants were divided into three groups according to their professional interests.

After welcoming the guests, the joint lecture part was started by the head of the hospital pharmacy, Dr. John. After the introduction, he showed the participants a short commented film about the history and present of VFN, the basic characteristics and definition of the activity and its broad scope in the field of treatment, science, research and teaching. Followed by Prof. Malík, deputy deputy for science, research and education, who talked about our hospital as a scientific and research institution, introduced the participants to a number of achievements and plans in this area. Preference 3.IK prof. Kršek presented the medical focus of his clinic to the participants, pointed out the excellence of his workplace and showed pharmacological intervention in the treatment of internal diseases using the example of Cushing's m.

In the next block, activities performed by pharmacists in VFN outside the hospital pharmacy were presented. Dr. Pilková dedicated her contribution to the work of the Department of Clinical Pharmacology and Pharmacy, where pharmacists work in the field of pharmacotherapy control, advisory services and individual consultations for outpatients. The participants of the meeting were introduced to the work of the Toxicology Information Center by Mgr. Běhounková She talked about consultations in the field of intoxication for the professional and lay public and about the provision of antidotes and anti-infectives in close cooperation with the hospital pharmacy.

The last part of the morning was started by Dr. Janů with a comprehensive audiovisual introduction to the complex of professional and scientific activities provided by the pharmacy and briefly presented its individual specialized workplaces. Dr. Davidová talked in detail about pharmacy care for our patients in the public parts of the pharmacy, highlighted our consultation activities. She continued with a contribution that mapped the possibilities of pharmacotherapeutic support for rare diseases and clinical evaluations. This part sparked a rich discussion of our scientific visit. Dr. Golianová, presented the concrete possibility of preparing a number of sterile medicinal forms.

In the afternoon, the first group moved to the representative Ceramic Lounge, built according to the design of the prominent architect Jan Kotěra. Here, a specialized meeting with experts from the field of clinical pharmacy and the Toxicology Information Center was prepared for the participants. In the first lecture, Dr. Hartinger five different case studies solved at the Department of Clinical Pharmacology and Pharmacy and five possible different types of interventions by the clinical pharmacist. Interventions included, for example, a change in pharmacotherapy, solving a pharmacokinetic interaction, the consequences of changing the antibiotic according to the antibiotic center's recommendation, and, for example, preventing an overdose of digoxin. In the second lecture of the afternoon program, Mgr. Běhounková about the work of the Toxicology Information Center. She chose examples of the most frequently asked questions, especially poisonings in children, especially with cleaning products, medicines, plants. The principles of first aid in case of intoxication were part of the lecture. In the last part, she presented the stock of antidotes and their mechanism of action. The presentation of the Toxicological Information Center ended with a tour of the consultation rooms, a demonstration of working with toxicological databases.

The second group of people interested in AIO parenteral nutrition and the preparation of sterile drugs met with doc. Novák from 4.IK, where the participants had the opportunity to see a patient with an inserted central venous catheter and a practical demonstration of the education of a patient starting self-application of home parenteral nutrition, and subsequently transferred to the department of preparation of sterile drugs in the hospital pharmacy, where foreign pharmacists had the opportunity to see the premises, where parenteral nutrition is prepared. 

The third group visited the Clinic of Pediatrics and Hereditary Metabolic Disorders and its laboratories. With Professor Kožich, she focused on an overview of therapeutic options for DPM. in the laboratory and learned about the possibilities of newborn screening. With Dr. Davidou discussed in more depth the role of the pharmacist in the care of patients with rare diseases.
